I happened to have a Norton 3.5 CD, which I loaded onto an external hard
drive containing a universal install of Mac OS 8.1 (so I can use it to boot
040, as well as PowerMacs), then patched Disk Doctor to v3.5.2. Booting from
the external drive still displayed the "the disk is unreadable, do you want
to initialize it?" message. I hit cancel (of course), Only the external
drive appeared on the Desktop. I launched Disk Doctor, which did show the
missing hard drive, then found & fixed a slew of problems. As to what caused
it, "Warn about improper shutdown" was checked, and I'm almost certain Drive
Setup v1.6.2 was applied prior to the 8.5.1 update. I did notice Virex was
at version 5.8.1 and remembered problems were reported in this list a while
back when copying to NT Servers, so I upgraded Virex to 5.9.1.
