    Jeff,

    Adobe Acrobat is all you need.... you simply create cells in the
    .pdf file... and it is into those cells that the recepient of the
    file types (on the screen) then prints out.

    Adobe Acrobat is available from all the usual warehouses for both Mac
    and Windows OS....
