If I'm not mistaken your version of Win95 will not recognize a volume of
over 2GB. Most likely you need to run FDISK and set your primary DOS
partition to 2GB, with other partitions however you prefer (one 2GB and
one 1GB, or two 1.5GB). FAT32 is required to format volumes over 2GB as
one partition; your Win95 only supports FAT16. After you're done with
FDISK, you can use FORMAT.COM from your Win95 boot floppy to format your
volumes, or sometimes Win95 Setup will do it for you.

Also if you can set your BIOS to auto-detect the drive's parameters, I
would enable that, rather than manually inputting them.

Was this the manufacturer of the drive or the computer? If it came from
the HD vendor, uninstall anything that you installed off that disk. If
your BIOS supports 5GB volumes you shouldn't need it. If the disk came
from your computer vendor, I'd probably check the BIOS to see if the CD's
drive letter is set there.
