What *specifically* happens? AND Is it ISA or Proxy 2.0 that's being used?
I've got 20 Macs getting Internet access via an ISA box with no probs.

Whilst Macs will work perfectly well as SecureNAT clients, it would be best
to set any of your Mac apps that need "normal" internet access to use a
SOCKS compatible server rather than straight proxy.

So in Internet prefs select the SOCKS option instead and don't forget to put
    1080 as the port number.

SBS 2000 server should only need to be a SecureNAT (i.e. default gateway set
to internal IP of ISA server).
